Post-University Matriculation Program

For the purpose of supporting students’ post-University of New Haven pursuits, the Accessibility Resources Center has initiated a Post-University Matriculation Program to assist students with providing documentation of a disability to institutions of higher education, testing agencies, and/or other organizations requiring the information for admission, certification or licensure, and/or employment. 

Information submitted to the ARC for the purpose of documenting a disability and other disability-related materials located in the student’s ARC Student File are considered confidential educational records. The ARC Student Files are not integrated with formal university records, but rather are maintained separately in the ARC. 

The ARC will, at the student’s request, forward a copy of his/her records (including the documentation of a disability) to an appropriate qualified professional and/or other agency/institution/organization at any time throughout his/her enrollment at the University, or within seven (7) years after he/she ceases to be matriculated at the University. 

Requesting information to be sent to another professional/agency/institution/organization:

Students will be required to complete a Release of Confidential Information to Appropriate Qualified Professional or other Agency/Institution/Organization form for each request in its entirety including contact information for an appropriate professional at the organization/agency. This form must be printed and signed. This form can be handed into the Accessibility Resources Center office in person, or faxed to: (203) 931-6082

Students must complete and sign any required paperwork from the organization/agency and submit it to the Accessibility Resources Center a minimum of two weeks prior to the submission deadline, scheduled test date or any organization/agency requirements to ensure its receipt at the organization/agency by the specified deadline.

A staff member of the Accessibility Resources Center will complete the paperwork, provide the required documentation and forward the completed packet of information to the requesting individual, agency or organization. Documentation can only be sent to students in instances when a testing agency requires the student to submit directly in digital format.

Failure to follow these procedures may result in delay, substitution, or denial of reasonable accommodations by the requesting individual, agency or organization.
