Medical Examination & Immunizations

According to University policy and Connecticut state law, ALL new students (undergraduate and graduate) must provide proof of several health-related items, including a health examination report (physical exam) within one year prior to the start of classes in the initial term of full-time enrollment, and documentation of required vaccination status.

Please read the following information carefully. For everyone’s protection, students may not move into residence halls or start classes without first submitting the required health documentation. Due to the busy nature of many physicians’ practices, we recommend contacting the office to obtain your immunization records and (if necessary) obtain needed boosters as soon as possible.

Required for New Students and All Student-Athletes

All new students must complete the Medical Information Form, which can be downloaded below. The Medical Information Form outlines all of the medical documentation required to become a student at the University of New Haven. You must submit all of the following, using the form linked below:

  • Necessary Insurance and HIPAA Information
  • Required Health Examination Report (physical exam)
  • Proof of Required Vaccinations
  • (Optional) Proof of Encouraged Vaccinations

Overview of Requirements

The following information is also available on the Medical Information Form, but is summarized here for new students.

Health Examination Report (Physical Exam)

It is mandatory that all students entering the University of New Haven have a completed Health Examination Report on file, thus enabling the Health Services staff to render optimum health care when needed. All students are required to complete the health history form prior to the beginning of classes in the initial term of full-time enrollment. An exam is not required annually, except for student-athletes and some specialized academic programs that require an annual physical.

In the past several years, outbreaks of vaccine-preventable diseases on college campuses throughout the United States have resulted in many lost school days, severe complications from the diseases, anxieties for students and their parents, and large expenditures of monies to contain these outbreaks. Compliance by each student with the pre-entrance immunization policy at the University of New Haven protects the student and the general University community. All students are required to complete the health examination report prior to the beginning of classes in the initial term of enrollment.

Required Immunizations

For your own safety and that of your classmates, you will not be permitted to register for classes or access your residence hall until the University’s Health Services Office receives proof of the following for its records:

  • The State of Connecticut requires ALL students to have the MMR vaccine (Measles, Mumps, and Rubella). Vaccines after December 1956 or given before the first birthday are not valid. MMRV is also acceptable.
  • The State of Connecticut requires ALL students to have the Varicella (Chicken Pox) vaccine. Two doses required or proof of history of disease, or blood test to prove immunity required. MMRV is also acceptable.
  • The State of Connecticut requires that all students living in university housing receive 1 injection of the Meningitis (MCV 4 Sero Groups, A,C,Y and W135) vaccine within five (5) years of enrollment. If you are planning on living in the residence halls or in university housing, you MUST have received this vaccination and be able to produce valid proof. Proof this vaccine is also required of all University of New Haven athletes, whether living on or off campus.

Medical Exemptions

If the required immunizations cannot be received due to medical conditions, a Medical Exemption Certificate should be filled out by a healthcare provider and submitted to the upload email for review. Health services will submit the required documents to the DPH portal as state law requires. Only medical exemptions will be accepted, as religious exemptions are not permitted per state law.

Optional for New Students

Unless required by a specific academic program, the following vaccinations are optional but strongly encouraged for all students:

  • Tetanus: TDAP or TD vaccine within 8 years of enrollment and updated every 10 years
  • Hepatitis B vaccine (3 dose series)
  • Hepatitis A vaccine (2 dose series)
  • Gardasil HPV vaccine (3 dose series)
  • Meningitis/Sero Group B vaccine series
  • Tuberculosis (TB) screening form: If you answer yes to any questions a PPD or QuantiFERON TB gold test is required within 12 months of enrollment.
  • For Fall 2022, the University strongly encourages ALL students to be fully vaccinated against COVID-19 via a WHO-approved COVID-19 vaccine.

Submitting Medical Documentation to the University

Students can submit documents to Health Services in person by visiting the Alexander W. Nicholson Jr. Health Center, located in Sheffield Hall (Ground floor, southeast corner). See this location on Map.

Documents that should be submitted include: Health Examination Report (Physical Exam Form), Insurance Company Card, Driver’s License/Passport/ID, HIPAA Release Form, and Immunization Records.

  • Submit copies of the form(s) in person. Do not submit original forms without making a copy.
  • ALL forms must be legible and include patient name and date of birth.
  • ALL vaccination records MUST be legible and include patient name, type of vaccine, and date(s) of vaccination.

All records must be manually reviewed and individuals should allow at least two (2) business days for this review to occur.

Students can submit documents to Health Services via fax to (203) 937-4099.

Documents that should be submitted include: Health Examination Report (Physical Exam Form), Insurance Company Card, Driver’s License/Passport/ID, HIPAA Release Form, and Immunization Records.

  • Please include a Fax cover sheet indicating the student’s name and total number of pages being transmitted via fax.
  • Be sure to fax every page – if you printed anything back-to-back, ensure the second side is faxed as well.
  • ALL forms must be legible and include patient name and date of birth.
  • ALL vaccination records MUST be legible and include patient name, type of vaccine, and date(s) of vaccination.

All records must be manually reviewed and individuals should allow at least two (2) business days for this review to occur.

Students can submit documents to Health Services via email to UNHhealthformuploadonly@ynhh.org.

Documents that should be submitted include: Health Examination Report (Physical Exam Form), Insurance Company Card, Driver’s License/Passport/ID, HIPAA Release Form, and Immunization Records.

  • Submit either a PDF of the form(s) or screenshot/take a photo of the form(s) and attach to the email.
  • ALL forms must be legible and include patient name and date of birth.
  • ALL vaccination records MUST be legible and include patient name, type of vaccine, and date(s) of vaccination.

All records must be manually reviewed and individuals should allow at least two (2) business days for this review to occur.

Authorization for Access/Release of Medical Information
For students who request their medical records and vaccinations. Please allow up to two weeks for requested records to be sent. State law requires that the University keep medical files for a period of seven years after a student has left the institution. After that time period, all records are destroyed. Please contact Health Services for further information regarding your medical records.
